Guidelines for Submitting Effective Reports

Stick to the Facts

Base your report on firsthand knowledge or direct observation. If you're reporting something you heard from someone else, clearly indicate that it's hearsay and provide the source when possible.

Provide Supporting Evidence

Attach any relevant documentation such as emails, messages, photos, or other evidence that supports your report. This helps our team thoroughly investigate the matter.

Remain Professional

Present your report in a professional manner. Use clear language and avoid inflammatory or accusatory tone. Focus on behaviors and actions, not personal opinions.

Identify the Involved Party

Clearly identify the person(s) involved in the misconduct, including their name, role, department, and how you know them. This helps ensure the correct person is investigated.
